Provider Directory Information
If you are a provider listed in the JDNA directory, we may collect and display information such as your name, business name, credentials, NPI, services offered, counties served, languages spoken, insurance accepted, Medicaid participation status, certifications, availability, contact information, website links, biography, headshot or logo, business address, and other listing details. Providers are responsible for keeping their listing information accurate and current.

When you use our website or digital tools, we may automatically collect IP address, browser type, device type, operating system, referring website, pages viewed, links clicked, visit date and time, approximate location based on IP, and website usage patterns through cookies, pixels, analytics tools, log files, and similar technologies.

With Providers You Choose or Request
We may share your name, due date, county, insurance plan, language preference, and service needs with a provider to determine whether they can support you.
With Health Plans and Care Management Teams
If you are seeking services through insurance, Medicaid, NJ FamilyCare, or a health plan referral, we may share information to support referral coordination, coverage navigation, payment, quality review, or care management.
With Referral Partners
We may share limited referral status updates such as: referral received, member contacted, unable to reach, provider matched, appointment scheduled, additional support needed, or no available provider identified. We limit information shared to what is reasonably necessary.
With Service Providers and Vendors
We may share information with trusted vendors including website hosting, form platforms, scheduling platforms, payment processors, email or messaging platforms, analytics providers, data storage, customer support tools, security vendors, and billing support. These vendors may only use information as needed to provide services to JDNA.

No Emergency Services
International Users
JDNA is based in the United States and our Services are intended for users located in the United States.
